Acute Kidney Injury secondary to Urosepsis

A 60yr old female presented to the OPD with chief complaints of pedal edema since 10 days, decreased urine output since 10 days and fever since 10 days. History of present illness: The patient was apparently asymptomatic 5 years back following which she was diagnosed with DM2 on checkup and on Teneligliptin 20 mg. In 2019 ( 2 years ago) she developed fever, shortness of breath and pedal edema and diagnosed with Acute kidney injury secondary to urosepsis and resolved conservatively after dialysis (4 sessions) Now presented with history of fever, high grade since 10 days, not associated with vomiting and loose stools . Patient complaints pedal edema bilateral and pitting type, with decreased urine output and burning micturition. History of past illness: Outside reports suggest acute kidney injury. 65 YEARS OLD FEMALE WITH SYNCOPE

This Elog reflects my patient-centered online learning portfolio. A 65 year old female, housewife, resident of Chinatunalgudam  presented to the casualty with loss of consciousness. HISTORY OF PRESENTING ILLNESS: Patient was apparently asymptomatic two days ago. Then,  after walking for a long time,  she had an episode of syncope, associated with sweating,   After gaining consciousness after approximately. Not associated with palpitations, flushing, chest pain, muscle weakness, slurring of speech, headache.  The patient was brought to the casualty and given medication (glucose saline) and recovered completely. A 25 year old Male with Shortness of Breath and Pedal edema since 10 days

This Elog reflects my patient centered online learning portfolio. This Elog was created under the guidance of Dr Naveen (Intern). This is the case of a 25 year old man. CHIEF COMPLAINT : Shortness of Breath  Pedal edema since 10 days HISTORY OF PRESENT ILLNESS : Patient was apparently asymptomatic 10 days back. & then he presented with worsening breathlessness 3 hrs post binge (90) whisky. His earlier events 9th class - Had an episode of SOB  and had to sit back while playing with copious amount of sputum with expectoration (yellowish color, non foul smelling).
